  • WKCE Braille/Large Print Test Books Order Form
    Please order by September 12, 2014.  If you are experiencing problems with the form please contact CTB at 800-282-2203
  • WAA-SwD Braille Test Books and Picture Descriptions - Braille materials for the WAA-SwD are produced on an as needed basis. Use of the Braille and picture description accommodations should be consistent with day-to-day instructional methods. Contact Kristen Burton at 608-267-3164 or kristen.burton@dpi.wi.gov for more information.

Accommodations Forms

  • Badger Exam Request for Accommodation Form - Any accommodation not in the Usability, Accessibility, and Accommodations Guidelines must be approved by DPI prior to use during the Badger Exam.  DPI staff will determine whether the requested accommodation impacts the validity of the test results and/or the construct the test is intended to measure.
  • Badger Exam Request for Print on Demand Accommodation - The Print on Demand feature is extremely rare instances (e.g. photosensitive epilepsy) where a student’s condition prevents him or her from accessing material online. This request must be approved by DPI prior to use during the Badger Exam. DPI staff will review the request and if approved, activate the accommodation in TOMS.
  • WKCE/WAA-SwD Request for Accommodation Form - Any non-standard test accommodation not on the Assessment Matrix must be approved by DPI prior to use during the WSAS Assessments. DPI staff will determine whether the requested non-standard accommodation impacts the validity of the test results and/or the construct the test is intended to measure.
